You may use this form to notify the Department and to request that the Business Licensing Division cancel your Basic Business License (BBL). Please note that upon submission of your cancellation request, the specified BBL will be cancelled and the authority granted therein is no longer valid. Once the BBL is cancelled, the license cannot be reactivated. If you wish to conduct this business activity at a later time, a new application, supporting documents/registrations and fees will be required.
